Service Description: Medical care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes nurse practitioner, nurses, social worker, mental health counsellor, respiratory therapist, occupational therapist and dietitian * services include:
  • hypertension program / 24 hours blood pressure monitoring
  • anticoagulation therapy monitoring program (INR/Coumadin)
  • cancer screening - cervical cancer
  • diabetes care
  • nutrition / dietitian services
  • seasonal flu vaccines
  • program for older adults – MINT memory clinic, cognitive assessments and interventions
  • occupational therapy
  • chronic disease and pain management
  • fall prevention and management
  • immunizations
  • Ontario Telemedicine Network (OTN) - dermatology
  • lung disease care - assessment, monitoring and treatment of respiratory and cardiorespiratory disorders (i.e., asthma, chronic obstructive pulmonary disease); spirometry testing
  • smoking cessation program
  • well baby well child program
  • social work / mental health counselling - individuals aged 16 and older who live with a mild to moderate mental health condition
  • cognitive behaviour therapy for insomnia
  • foot care
