Service Description:

Medical care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes nurse practitioner, nurses, social worker, mental health counsellor, respiratory therapist, occupational therapist and dietitian * services include:
- hypertension program / 24 hours blood pressure monitoring
- anticoagulation therapy monitoring program (INR/Coumadin)
- cancer screening - cervical cancer
- diabetes care
- nutrition / dietitian services
- seasonal flu vaccines
- program for older adults – MINT memory clinic, cognitive assessments and interventions
- occupational therapy
- chronic disease and pain management
- fall prevention and management
- immunizations
- Ontario Telemedicine Network (OTN) - dermatology
- lung disease care - assessment, monitoring and treatment of respiratory and cardiorespiratory disorders (i.e., asthma, chronic obstructive pulmonary disease); spirometry testing
- smoking cessation program
- well baby well child program
- social work / mental health counselling - individuals aged 16 and older who live with a mild to moderate mental health condition
- cognitive behaviour therapy for insomnia
- foot care
